What the registration asks for
- Owner name, phone number and email address
- Your home address
- Your dog's name and breed
- Your dog's age in years
- Any special health or grooming needs

Send your details and your dog’s here. The salon will confirm by phone. Vaccination records (Rabies, Distemper and Parvovirus) go to shearblisspet@aol.com or by photo on Facebook Messenger.

Service authorization
By submitting the registration, you authorize Shear Bliss Pet Salon to perform grooming services for your pet.
Pet comfort and safety
The salon takes every precaution to keep the room safe and calm, especially for senior pets and pets with special health needs. You acknowledge that grooming can occasionally uncover or aggravate a pre-existing health condition.
Emergency care
If there is a medical issue or emergency, you give the salon permission to seek immediate veterinary care at the owner's expense. You release the salon and its team from liability regarding accidents or pre-existing conditions.
Matted coats
Severely matted coats need extra time and specialized care, which brings additional fees. The team works with extreme care, and matting removal carries an inherent risk of minor skin irritation or nicks.
Agreement
Submitting the registration acts as your agreement to the salon's General, Matted Pet and Senior Pet Release policies.
